With the ability to combine the intensity of inpatient therapy with the specialization of outpatient services, the use of rehabilitation at home has increased dramatically over the past decade. At the VNA, our rehab department is the largest in Western New York, and boasts some of the most experienced clinicians in the area. We treat our patients where they most prefer to be, in their own homes.

Physical Therapy Physical therapists (PTs) are health care professionals who diagnose and treat individuals of all ages, from newborns to the very oldest, who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives.
Occupational Therapy Occupational therapist provide skilled treatment that helps individuals achieve independence in all facets of their lives. Occupational therapy assists people in developing the "skills for the job of living" necessary for independent and satisfying lives.
Speech/Language Pathology Speech therapists evaluate, diagnose and treat speech, language, cognitive-communication and swallowing disorders in individuals of all ages, from infants to the elderly.
Medical Social Work Medical social workers provide psychosocial support to people, families, or vulnerable populations so they can cope with chronic, acute, or terminal illnesses. They also advise family caregivers, counsel patients, and help plan for patients’ needs after discharge from home health care.
Nutritional Therapy Registered dieticians provide reliable, objective nutrition information, separate facts from fads and translate the latest scientific findings into easy-to-understand nutrition information. They work closely with other members of our team, especially with diabetic patients, and those receiving nutritional intake from alternative sources.

At the VNA, our not-for-profit status allows us to re-invest in the latest, most effective therapies to improve the recovery of our patients. We constantly monitor the rehabilitation literature to identify the newest, research driven therapies, and apply them to the home setting. Some of the specialty areas in which we are highly proficient include:
Falls Risk Assessment and Intervention An evidence-based falls risk assessment is included with every patient evaluation we perform. We use objective, well researched tools and interventions to track and improve the ability of our patients to safely walk and function within their home environment. We also provide an extensive home evaluation and offer a comprehensive list of recommendations to improve the safety of our patients as they progress throughout their daily activities.
Total Joint Replacement Fast-track A program which allows patients who have had a total knee or hip replacement to return home straight from the hospital after their surgery. Our team is dedicated to providing initial visits the day after you return home, and in some cases, a pre-operative visit may be arranged to prepare you in advance for your return. Therapy frequencies of up to 6 days per week are possible, and you will benefit from as many of the comprehensive VNA services as you need.
Vestibular Dysfunction and Intervention Program A specific physical therapy treatment program for dizziness caused by inner ear dysfunction.
Osteoporosis and Compression Fracture Management A program of progressive exercises and positioning recommendations specifically designed to decrease pain, improve posture, modify risk factors, and increase mobility in the population with this debilitating condition.
Advanced Multidisciplinary Wound Care A physical therapy treatment program led by a Certified Wound Specialist which may utilize electrical stimulation, conservative sharp debridement, positioning and education to promote healing of chronic and difficult to heal wounds.
Low Vision Assessment and Intervention An occupational therapy program which includes adaptive techniques for functioning independently with low visual acuity. Instruction in innovative techniques for diabetic and ostomy care for patients with low vision is also available.
Vital Stim™ therapy A speech therapy program where specifically calibrated electrical stimulation is utilized along with targeted exercises to improve the ability to swallow. This therapy has demonstrated results which include the complete return to normal swallowing function in patients who have relied on feeding tubes for years to obtain their nutritional intake.
The Rehabilitation Department at the VNA of WNY has the added advantage of being able to coordinate care with all of the additional services offered by the VNA. Whether it is Home Nursing, Respiratory and Home Oxygen services, Home Health Aid services, or Home Infusion services, VNA therapists work side by side with hundreds of other health care professionals, all dedicated to improving the health of our community.
