Our nurses began their mission in Western New York homes in 1885, and over a century later, skilled service remains the heart and soul of our organization. Working under the direction of the referring physician, our team of nurses, physical therapists, occupational therapists, speech therapists, respiratory therapists, medical social workers, and nutritionists are uniquely committed to high quality outcomes and superior patient satisfaction.
Medical Supervision: All services are provided under a plan of care established and monitored by the patient’s physician.
Disease Management and Clinical Path Implementation: Focusing on education, nutrition, patient compliance, and communication, our disease management programs help to control the disease process and maximize wellness. The goal is to provide a template of care that can be modified based on individual patient needs. Each path identifies key goals and actions to meet those goals as well as specific teaching to be provided. Patients can often be effectively managed once stabilized via routine phone management. This approach has resulted in reduced hospitalizations and emergency room visits. All paths are developed utilizing an interdisciplinary approach involving all key disciplines. Current paths include diabetes, congestive heart failure, pneumonia, DVT, asthma, COPD, and IDDM. Our care paths are your path to independence.
Nursing: Our home health RNs assess and care for patients of all ages and conditions. Their services included patient observation and assessment, patient and family teaching, the administration of medications, wound care, tracheotomies, gastrostomy tubes and foley catheters, as well as an assortment of other treatments as ordered.
Our size enables us to specialize, and we have established specialty teams in infusion therapy, mental health, obstetrics as well as a dedicated pediatric team.
Physical Therapy: Our PTs assess the rehabilitation needs of patients and establishes realistic goals toward recovering function. Services provided in the home include therapeutic exercise, gait training, range of motion, and other modalities. Pediatric therapists are available for the special needs of the child and the family.
Occupational Therapy: These therapists assess the patient’s ability to to carry out activities of daily living and develop treatment plans; they included training in feeding, dressing, balance, assistive devices, and other interventions.
Speech/Language Pathology: After assessment by these professionals, treatment for speech, language, communication disabilities and swallowing disorders are implemented in the home.
Medical Social Work: MSWs assess and attempt to improve the patient’s emotional adjustment by counseling, referral to community agencies, helping with finances, and other matters.
Respiratory Care: Our Respiratory Care practitioners provide education and compliance monitoring of oxygen, suctioning equipment, nebulizers, home ventilators, CPAP, BiPAP, and many other types of respiratory equipment. Again, pediatric specialists are available for that special-needs population.
Nutritional Services: Our registered dietician provides assessment and patient counseling concerning nutrition and therapeutic diets. The dietitian also serves as a resource to other professional staff. |
