  Diabetes Program: Overview

Why Choose the VNA for Diabetes Care?

With a team of dedicated nurses who specialize in the care and management of diabetic patients in the home care setting and an extensive offering of more than 50 products and services, we offer diabetes care that is second to none.

Our comprehensive diabetes program was developed in collaboration with the Diabetes-Endocrinology Center of Western New York. It is specifically tailored to provide care in patients' homes and is based on the standards of care established by the American Diabetes Association. Our Diabetes Program also follows the Western New York Collaborative Diabetes Guidelines for Adult Care.



Our Approach to Care

Patients are required to complete an in-depth assessment of their physical, psychosocial and environmental health status with a qualified healthcare professional to identify areas in need of intervention by the home care team. Once that is accomplished, our team works closely with the physician to develop and implement an individualized plan of care for the patient.

Our specialists are exceptionally competent in teaching patients with diabetes about the important role that they can play in self-management of their diabetes, including symptom management, medication management, insulin preparation and administration, blood glucose montoring, nutrition, foot/skin and oral care and exercise.

Our goal is to assist patients in living with -- and managing -- the challenging demands of diabetes to help them achieve the best quality of life possible.

  • Anodyne Therapy System using infrared energy for treatment of neuropathy
  • Physical therapy for falls reduction, balance improvement and improvement in activity tolerance
  • Occupational therapy focusing on independence through the use of adaptive equipment
  • Medical social workers and nutritionists
  • Annual immunizations against flu and pneumonia
  • Lab work, including A1C levels, fasting lipid profiles and others by request
  • Expertise in the treatment of diabetic skin conditions
  • Ongoing communication with the patient's physician
  • In-house pharmacy and nursing services for treating infections
